Job Title: Patient Care Coordinator

Status: Full Time, Non-Exempt

Reports To: Call Center Team Captain

Department: Call Center

Location: Flint, MI

 

Position Overview:

Acts as a liaison between the Patient and the Care Team (prescriber, medical assistant, nurse, etc.). Ensures patient has a clear understanding of the importance of therapy adherence and what effects the prescribed medication may have.

 

Education / Licensing Requirements:

High School Diploma or GED required, Associates Degree preferred.

Pharmacy Technician Certification preferred.

 

Experience Requirements:

1-3 years of customer service experience

Responsibilities

  • Communicates with patient (or their approved authorized representative) to confirm shipping details.

  • Schedules patients' medication shipments.

  • Reiterates with patient prescription information and dosage.

  • Ensures necessary ancillary items such as antiseptic swabs and needles are included on the order.

  • Counsels patient regarding therapy adherence.

  • Utilizes company software to obtain payment for copay.

  • Obtains and updates patient profile with new insurance information as needed.

  • Maintains accurate history notes and records in the patient profile.

  • Sends Patient Status Reports to physician throughout the care process.

  • Reports all customer complaints according to company policies and procedures.

  • Assists in streamlining communication with the patient, insurance carrier, pharmacist, nurse, and physician.

  • Perform all tasks in a manner that is consistent with corporate policies as well as state and federal laws.

  • Knowledgeable regarding Senderra’s community values and demonstrates them daily.

  • Other duties as assigned.

 

Working Conditions

  • Requires normal hearing levels (corrected).

  • Requires working at a desk to use a phone and computer for extended periods of time. Requires sitting, bending.

  • Requires normal finger dexterity for keyboarding.

  • Works effectively with frequent interruptions.

  • Lifting requirements of 15 lbs. occasionally.

  • Minimal travel <5%.
